Serenity by Nexiva is a connected mental-health care platform for therapists, clinics, participants, and care teams. It captures the work that happens between therapy sessions and turns it into structured, therapist-ready context for better care delivery.
The problem we solve:
Therapy is one hour inside a 168-hour week. Between appointments, participants process difficult moments, practice coping skills, avoid homework, track moods, notice relationship patterns, and make progress — but most tools don't capture this work in a way therapists can use. Therapists spend the first minutes of each session reconstructing what happened. Clinics lack visibility into engagement. Participants stare at blank journals, unsure where to start.
How Serenity works:
Participant workspace: 40+ guided clinical tools spanning CBT (thought records, cognitive distortions, putting thoughts on trial), DBT (behavior chain analysis), anxiety management (GAD thought records), relapse prevention (cravings diaries), relationship health (22 relationship worksheets covering communication, authenticity, codependency, connection rituals, values alignment, and Imago work), positive psychology (gratitude, resilience planning, values surveys, energy management), mindfulness (full 8-week MBSR workbook), and care planning (mental health CV, triggers and glimmers). All tools include voice input support, UI translation, and guided prompts that reduce blank-page stress.
Therapist command center: Piko, Serenity's RAG-powered AI assistant, synthesizes participant journals, worksheet responses, mood patterns, session transcripts, and clinical notes into session briefs in under 5 seconds. Therapists generate clinical documentation in 6 standardized formats (SOAP, DAP, BIRP, GIRP, PIRP, SIRP) plus intake summaries, discharge summaries, and parent consultation notes — all starting from structured drafts with mandatory clinician review before finalization. Treatment plans link presenting concerns to goals, interventions, and assigned tools. Homework adherence tracking captures completion status, difficulty, barriers, therapist feedback, and adjustment plans.
Practice operations: Public therapist microsites with booking flows, staged lead-to-participant pipeline (request → onboarding → approval → active care), video sessions (Agora-powered), intake form builder, invoicing, payment tracking, team workspace with role-based permissions (owner, admin, therapist, supervisor, intern), and supervision/case review workflows.
Clinical safety and trust: Rule-based risk scanning detects self-harm, suicide ideation, abuse, violence, and severe distress signals in participant entries — flagging for therapist review, never diagnosing. Consent management tracks AI documentation consent, telehealth consent, data sharing consent, and guardian consent with immutable audit history. All AI outputs carry "draft" status until therapist approval. 520+ lines of security rules enforce role-based access at every data layer.
Origin: Serenity started as one digitized CBT worksheet. It has grown into a full care operating system for modern mental-health practice.
